Role: Lead UX Designer

Discharge tracker solves the problem of late and unorganized patient discharge in hospitals. This tool / app was designed specifically for discharge nurses to manage identify and overcome discharge barriers for their patients. My role on this project was system design, information architecture, content management to capture the users' experience. I also was partially responsible for designing the interface.
